Information you submit is used solely to respond to your inquiry, evaluate your consultation request, and communicate with you about your care at Sovereign Medicine.

Once you become a patient of Sovereign Medicine PLLC, your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA). Our Notice of Privacy Practices, provided at the time of enrollment, governs how we handle your protected health information.

Information submitted through pre-enrollment inquiry forms (such as consultation request forms) is not yet subject to HIPAA but is treated with the same standard of confidentiality.

Our consultation request form is hosted by Google Forms. Submission of that form is subject to Google's Privacy Policy. Our established patient portal is hosted by Airform. Use of that portal is subject to Airform's applicable terms.

We are not responsible for the privacy practices of third-party platforms linked from this site.

We retain inquiry information for a reasonable period to respond to your request and maintain records of communications. Patient health records are retained in accordance with applicable Tennessee state law and federal regulations.

You may request access to, correction of, or deletion of personal information you have submitted to us by contacting us at [email protected]. Requests related to protected health information as a patient are governed by our HIPAA Notice of Privacy Practices.

This website is not directed to children under the age of 13. We do not knowingly collect personal information from children under 13. Sovereign Medicine accepts patients ages 16 and older.

We may update this Privacy Policy from time to time. When we do, we will revise the effective date below. Continued use of this site after changes are posted constitutes your acceptance of the updated policy.
